For grammar consistency Category:Page Disambiguous should be Category:Page Disambiguation, and it sounds better. Otherwise the proposed category structure looks pretty good. Does the Category Portal play into this at all?

As for page names and file names, I think they might need an initial cap, in fact the wiki enforces it for pages. Templates usually call images based on the page name, so whatever naming convention is used for those two, they should be the same. By the way, will existing non-conforming files be expected to be changed?
